4.5.4-11-4 Ensure on-orbit anomalies are reviewed for reach-back Phase B | Phase C | Phase D1 | Phase D2 | Phase D3 | Ensure that anomalies from both in-system (common program) and cross-systems (supporting or parallel programs and services) are evaluated and considered in the event of on-orbit anomalies. Root cause and disposition of the anomaly must consider reach back, reach across and reach forward. Root cause and final disposition must be documented into a lessons learned database as appropriate to the security level of the anomaly and program.
